Diabetes is a chronic disease that originates because the pancreas does not synthesize the amount of insulin that the human body needs, makes it of a lower quality or is not able to use it effectively.  Insulin is a hormone produced by the pancreas. Its main function is the maintenance of the appropriate blood glucose values. It allows glucose to enter the body and be transported into the cells, where it is transformed into energy for muscles and tissues to function. In addition, it helps cells store glucose until use is necessary.  In people with diabetes there is an excess of blood glucose (hyperglycemia), as it is not distributed properly. Noemí González, secretary of the Spanish Diabetes Society (SED) and specialist in Endocrinology and Nutrition at hospital La Paz, Madrid, explains that high glucose can be harmful “to the whole body, but mainly to the heart, kidney and arteries, so people who have diabetes and do not know it or do not treat it have a better risk of kidney problems , heart attacks, vision loss and lower limb amputations.”

According to the di@bet.es study, the incidence of diabetes in Spain is 11.58 cases per 1,000 people per year. That is, 1,057 new cases occur every day. The current percentage of people with diabetes (prevalence) according to this same study is 13.8 percent.

Causes

Type 1 diabetes

It usually occurs in children, although it can also be started in adolescentse and adults. It usually occurs abruptly and often regardless of family history.

Destruction of the cells that produce insulin in the pancreas (beta cells) occurs by autoantibodies.  The initial mechanism that induces the appearance of these antibodies is not fully identified and very complex. It is investigated whether the origin is, according to Avila, in “a genetic predisposition that, due to different ambiantial factors, produces that autoimmune response that destroys those cells”.

Type 2 diabetes

It arises in adulthood, its incidence increases in the elderly and is about ten times more common than type 1. It results in a decrease in the action of insulin, so that, although there is a lot, it cannot act. Gonzalez states that “a mixed component is given: on the one hand, there is less insulin in the pancreas and, on the other hand, this insulin works worse in tissues (so-called insulin resistance)”.

“Its main cause is obesity because fatty tissue produces certain substances that decrease the sensitivity of insulin receptors,” avila adds. Since obesity has grown very significantly in Spain, so has this type of diabetes.

Gestational diabetes

During pregnancy insulin increases to increase energy reserves. Sometimes this increase does not occur, which can lead to gestational diabetes. It usually disappears after childbirth, but these women are at high risk of developing type 2 diabetes throughout their lives.
